Our New Patient Screener is for anyone new to our services at Anxiety Specialists of Atlanta or anyone returning to renew services after at least a one (1) year gap of being seen by someone on our team. The screener should only take a few minutes to complete.

The safety and security of your information, even before you become a patient with us, is of the utmost importance. This is why we have developed a new screening system IntakeQ, a software company that aims to empower practices with innovative, user-friendly software that simplifies operations, improves client communication, and ultimately enhances the patient experience, all while maintaining the highest levels of patient healthcare information security.

This screening process is in response to the high volume of requests we get for new patient appointments. We believe this briefer screener will increase our efficiency and accuracy in determining whether our clinic is an appropriate fit for your needs. We will do our best to match you with a therapist whose background and availability address the information you provide. If we are unable to do so, we will do our best to provide you with appropriate referral resources.
